In September 2023, the National Institutes of Health designated people with disabilities to be a “health disparities” population.

 

This validation of the lived experience of individuals with disabilities is a wake-up call for health plans, health systems, developmental disability providers, government agencies and others who will need to acknowledge and incorporate this designation into their programs, policies, and operations.

 

People with intellectual and developmental disabilities (IDD) constitute a unique  cohort of the disabled population and have distinctive patterns of risks and conditions that effect their quality of health, quality of life and life-course outcomes.  These need to be reflected in their care planning and care delivery.

We take “Triple Aim” at IDD; improving both the quality of care and quality of health of the IDD community requires orchestrating and optimizing goal-directed collaboration, operating relationships, and knowledge management between the programs, platforms, partners, and personnel that compromise the IDD systems of care

 

We understand and can help you meet the functional requirements of a scalable and sustainable IDD-Tailored Population Health Management infrastructure, and help organizations design and implement an approach to each requirement. Through the identification and prioritization of the core goal-directed competencies, we focus on immediate term operational performance initiatives and help organizations strengthen their position to negotiate on ‘value’ of future contracts.

 

Organizational Capacity Assessment

  • An organization’s journey to a successful IDD-focused population health management strategy and plan spans across a number of systems and programs. We provide clients with insights and direction on their capacity for systems-based practice, population management, and accountable care while helping them map out their journey from Vision to Value.

Population Health Readiness

  • Determining your approach to IDD-Tailored population health requires a commitment from a number of resources and teams across and organization. Prior to finalizing an approach or path, we provide clients with a current state gap analysis and road map to achieve client-specific goals for systems-based practice and future fee-for-value contracting.

Implementation & Interaction Design

  • With a goal of delivering directed interactions between people, platforms, programs, and partners, we develop a client-centered implementation plan that spans across the necessary functional requirements and/or the associated operational capabilities to help clients successfully achieve their intended objectives.

Program Development & Program Management Support

  • From the strategy to the implementation, having a strong program management and support staff can keep the project and team moving towards the set goal. We help clients in the development of programs, initiatives and/or service elements to support population management or value-based care goals by providing hands on program management, implementation, and go-live support.

Steven Merahn, MD

Founder and Chief Executive Officer 

​Dr. Merahn is an experienced physician executive with a remarkably diverse career across almost the entire healthcare ecosystem, including clinical practice, public health, hospital and integrated delivery networks, health plans and accountable care, as well as clinical analytics, care coordination, strategic communications and healthcare information services. A recognized thought leader in autism, IDD, complex care, population health management and systems-based practice, he has been a vocal advocate for the value of the 'human-factor' in care delivery, clinical quality and health status improvement.  Dr. Merahn has served in strategic, operational, and clinical leadership roles in organizations serving medically complex and socially disadvantaged communities, most recently as Chief Medical Officer for a national autism services provider and as Medical Director for two organizations serving over 30,000 people with IDD.
